Health Insurance: 7 Required Coverages

Switzerland operates one of the most comprehensive mandatory health insurance systems in the world. Under the KVG (Krankenversicherungsgesetz) / LAMal (Loi sur l'assurance-maladie), every resident of Switzerland must purchase basic health insurance (Grundversicherung / assurance de base) from one of approximately 50 approved insurers. Major carriers include Helsana, CSS, Swica, Sanitas, Concordia, and ÖKK. Premiums vary by canton, age, and deductible level (franchise), with average monthly premiums exceeding CHF 450–650 per adult. Coverage includes hospitalization, outpatient care, prescriptions, maternity, and mental health.

Swiss KVG/LAMal coverage is strictly territorial — it applies to treatment received in Switzerland. When a Swiss national moves to Colombia and deregisters from Switzerland (Abmeldung), their KVG coverage ceases upon departure. The insurer must be notified of the departure date.

Tax Considerations

There is no double taxation treaty between Switzerland and Colombia. Swiss nationals moving to Colombia must manage their tax obligations in both jurisdictions independently.

Swiss tax exit procedure. Swiss nationals who leave Switzerland permanently must formally deregister (Abmeldung) with their commune of residence. Swiss cantonal and federal income tax is based on residence — upon successful deregistration, Swiss tax residency ends. However, Swiss nationals retain a Swiss tax connection if they maintain a permanent home (Wohnsitz) in Switzerland or if a Swiss spouse or minor children remain registered there. Swiss withholding tax (Verrechnungssteuer) at 35% applies to Swiss investment income (dividends from Swiss companies, Swiss bank interest) regardless of where the recipient lives, and must be reclaimed — partially, where no treaty applies — through a complex refund procedure with ESTV (Eidgenössische Steuerverwaltung / AFC).

Without a CDI between Switzerland and Colombia, Swiss withholding tax on Swiss-source dividends and interest (35%) cannot be offset against Colombian tax through treaty coordination. Swiss nationals with Swiss investment income should seek advice from a cantonal tax specialist before completing the Abmeldung and establishing Colombian tax residency.

Colombian tax residency. Colombia taxes residents on worldwide income after five years of continuous residence (Law 2010/2019 new-resident exemption applies for the first five years). Swiss nationals who become Colombian tax residents after the exemption period must declare worldwide income including AHV pension disbursements, Swiss rental income, and Swiss private pension (pillar 2 / BVG) income.

Document Legalization

Switzerland joined the Hague Apostille Convention in 1973. Apostilles are issued by the Cantonal Chancelleries (Staatskanzleien / Chancelleries d'État) for documents issued by cantonal authorities (including most criminal records), and by the Federal Chancellery for federal documents. An online Apostille service (eApostille) is available for cantonal documents in some cantons. Criminal record extracts from CASREC (the federal criminal records office) are issued by the Bundesamt für Justiz / Office fédéral de la justice and require a separate Apostille from the Federal Chancellery.

Strafregisterauszug (Criminal Record) — Step by Step

  1. Request the Strafregisterauszug (German) / Extrait du casier judiciaire (French) / Estratto del casellario giudiziario (Italian) from the Strafregister-Informationssystem VOSTRA online portal (identification via Swiss ID or SuisseID), or in person at the Federal Office of Justice (Bundesamt für Justiz), Bern.   2. Obtain the Apostille from the Federal Chancellery (Bundeskanzlei / Chancellerie fédérale). Applications can be submitted online or by post. Cost: CHF 17 per document. Processing: 2–4 business days.
  3. Certified Spanish translation. Swiss criminal records issued in German, French, or Italian must be translated into Spanish by a certified translator authorized by the Colombian Cancillería.
